Predictive value of NLR combined with serum LDL-C/HDL-C, Lp-PLA2, and D-dimer for early neurological deterioration in patients with cerebral infarction

Abstract:

Objective To investigate the predictive value of the neutrophil-to-lymphocyte ratio (NLR), low-density lipoprotein cholesterol (LDL-C)/high-density lipoprotein cholesterol (HDL-C), lipoprotein-associated phospholipase A2 (Lp-PLA2), and D-dimer (D-D) for early neurological deterioration (END) in patients with cerebral infarction (CI). Methods The clinical data of 302 patients with CI admitted to our hospital from December 2022 to December 2025 were retrospectively analyzed. According to the National Institutes of Health Stroke Scale (NIHSS) score, the patients were divided into a deterioration group (n=81) and a favorable group (n=221). General clinical data and laboratory indicators were compared between the two groups. Independent predictors of END in patients with CI were identified, the relationships between NLR, LDL-C/HDL-C, Lp-PLA2, D-D, and neurological function were analyzed, and the predictive value of NLR, LDL-C/HDL-C, Lp-PLA2, and D-D for END was explored. Results Multivariate Logistic regression analysis showed that higher LDL-C/HDL-C, Lp-PLA2, D-D, NLR, and NIHSS score were all independent predictors of END in patients with CI (P<0.05). Each of these laboratory indicators could effectively predict END, and the combined predictive value of the four markers was superior (P<0.05). Conclusion NLR, LDL-C/HDL-C, Lp-PLA2, and D-D are closely associated with END in patients with CI, and their combined application may assist clinical prediction.
