肺癌放化疗患者焦虑、抑郁共病与单一情绪障碍的危险因素分析

Risk factors for comorbid anxiety and depression and for a single emotional disorder in patients with lung cancer receiving chemoradiotherapy

摘要:

目的 分析肺癌放化疗患者焦虑、抑郁共病与单一情绪障碍的危险因素。方法 选取漯河市中心医院2022年9月-2025年9月肺癌放化疗患者108例。收集患者的社会人口学资料、临床特征、治疗相关因素以及心理与社会因素。在放化疗2周后评估患者焦虑、抑郁情况,并将同时存在焦虑和抑郁症状的患者纳入共病组(n=42),有单一情绪症状的患者纳入单一症状组(n=40),其余纳入无症状组(n=26)。用多分类logistic回归分析探究情绪障碍的独立危险因素。结果 伴有焦虑和抑郁情绪患者42例,单纯焦虑或抑郁患者40例,无症状患者26例。3组的文化程度、个人月收入、临床分期、领悟社会支持量表(PSSS)评分、不良反应分级、匹兹堡睡眠质量指数(PSQI)评分差异均有统计学意义(P<0.05)。Logistic分析显示,个人月收入<3 000元、临床分期Ⅲ~Ⅳ期、PSSS评分<40分、PSQI评分>7分是肺癌放化疗患者发生单一情绪障碍的独立危险因素(P<0.05);个人月收入<3 000元、临床分期Ⅲ~Ⅳ期、PSSS评分<40分、不良反应分级3~4级、PSQI评分>7分是肺癌放化疗患者焦虑、抑郁共病的独立危险因素(P<0.05)。结论 较低个人月收入、高临床分期、低社会支持、高不良反应分级以及睡眠障碍是肺癌放化疗患者焦虑、抑郁共病的危险因素,临床需对高危人群进行早期识别,并实施综合性干预措施,以改善患者身心健康。

Abstract:

Objective To analyze the risk factors for comorbid anxiety and depression and for a single emotional disorder in patients with lung cancer receiving chemoradiotherapy.Methods A total of 108 patients with lung cancer who underwent chemoradiotherapy at Luohe Central Hospital from September 2022 to September 2025 were enrolled. Sociodemographic data, clinical characteristics, treatment-related factors, and psychological and social factors were collected. Anxiety and depression were assessed 2 weeks after chemoradiotherapy. Patients with both anxiety and depression symptoms were assigned to the comorbidity group (n=42), those with only one emotional symptom were assigned to the single-symptom group (n=40), and the remaining patients were assigned to the asymptomatic group (n=26). Multinomial logistic regression was used to identify independent risk factors for emotional disorders.Results There were 42 patients with comorbid anxiety and depression, 40 patients with either anxiety or depression alone, and 26 asymptomatic patients. Significant differences were observed among the three groups in educational level, personal monthly income, clinical stage, perceived social support scale (PSSS) score, adverse reaction grade, and Pittsburgh sleep quality index (PSQI) score (P<0.05). Logistic regression analysis showed that personal monthly income <3 000 yuan, clinical stage Ⅲ-Ⅳ, PSSS score <40, and PSQI score >7 were independent risk factors for single emotional disorder in patients with lung cancer receiving chemoradiotherapy (P<0.05). Personal monthly income <3 000 yuan, clinical stage Ⅲ-Ⅳ, PSSS score <40, adverse reaction grade 3-4, and PSQI score >7 were independent risk factors for comorbid anxiety and depression (P<0.05). Conclusion Lower personal monthly income, advanced clinical stage, low social support, more severe adverse reactions, and sleep disturbance are risk factors for comorbid anxiety and depression in patients with lung cancer receiving chemoradiotherapy. Early identification of high-risk patients and comprehensive interventions are needed to improve both physical and mental health.
