NLR联合血LDL-C/HDL-C、Lp-PLA2、D-二聚体对脑梗死患者早期神经功能恶化的预测价值

Predictive value of NLR combined with serum LDL-C/HDL-C, Lp-PLA2, and D-dimer for early neurological deterioration in patients with cerebral infarction

摘要:

目的 探究中性粒细胞与淋巴细胞比值(NLR)、低密度脂蛋白胆固醇(LDL-C)/高密度脂蛋白胆固醇(HDL-C)、脂蛋白相关磷脂酶A2(Lp-PLA2)、D-二聚体(D-D)对脑梗死(CI)患者早期神经功能恶化(END)的预测价值。方法 回顾性分析2022年12月至2025年12月我院收治的302例CI患者临床资料,依据美国国立卫生研究院卒中量表(NIHSS)评分分成恶化组(n=81)与良好组(n=221),比较两组临床一般资料、实验室指标,筛选CI患者END的独立预测因子,分析NLR、LDL-C/HDL-C、Lp-PLA2、D-D与神经功能的关系,探讨NLR、LDL-C/HDL-C、Lp-PLA2、D-D对END的预测价值。结果 经多因素logistic回归分析,LDL-C/HDL-C、Lp-PLA2、D-D、NLR及NIHSS评分高均为CI患者END的独立预测因子(P<0.05),上述实验室指标均可有效预测END,但四者联合预测的价值更优(P<0.05)。结论 NLR、LDL-C/HDL-C、Lp-PLA2、D-D与CI患者END密切相关,且四者联合可辅助临床预测。

Abstract:

Objective To investigate the predictive value of the neutrophil-to-lymphocyte ratio (NLR), low-density lipoprotein cholesterol (LDL-C)/high-density lipoprotein cholesterol (HDL-C), lipoprotein-associated phospholipase A2 (Lp-PLA2), and D-dimer (D-D) for early neurological deterioration (END) in patients with cerebral infarction (CI). Methods The clinical data of 302 patients with CI admitted to our hospital from December 2022 to December 2025 were retrospectively analyzed. According to the National Institutes of Health Stroke Scale (NIHSS) score, the patients were divided into a deterioration group (n=81) and a favorable group (n=221). General clinical data and laboratory indicators were compared between the two groups. Independent predictors of END in patients with CI were identified, the relationships between NLR, LDL-C/HDL-C, Lp-PLA2, D-D, and neurological function were analyzed, and the predictive value of NLR, LDL-C/HDL-C, Lp-PLA2, and D-D for END was explored. Results Multivariate Logistic regression analysis showed that higher LDL-C/HDL-C, Lp-PLA2, D-D, NLR, and NIHSS score were all independent predictors of END in patients with CI (P<0.05). Each of these laboratory indicators could effectively predict END, and the combined predictive value of the four markers was superior (P<0.05). Conclusion NLR, LDL-C/HDL-C, Lp-PLA2, and D-D are closely associated with END in patients with CI, and their combined application may assist clinical prediction.
